Our team has a wide knowledge of government and community services. They are able to respond to enquiries on a number of subjects and if they are unable to answer the question directly they will refer you to the right agency. We maintain links with other social service providers in the area to ensure we stay up to date.
If there is a situation causing you concern, we are available for, advice, advocacy, assistance, referral and support. The concern may be related to a health or social need.
Our team takes a holistic approach to the welling of older people and support their physical, emotional, cultural and mental health needs.
This service aims to maximise independence, and enhance the agility of older people to overcome challenges by providing short term support from our team, or coordinated support.

We understand that sometimes challenges can be stressful and overwhelming, and we will tailor support offered to suit individual needs of the older person to maximise independence in overcoming these challenges.
Our experienced nurse or social worker will assess and may visit you or the person involved and help them decide a way forward.
The Community Health Team often serve as ‘navigators’ through the complex healthcare and social service systems, ensuring that older people have access to services and information.
The team have a wide knowledge of the needs of older people and their carers and work closely with other community based organisations and health professionals who also provide support and services.
